Community Mental Health implemented a state-wide project of metabolic monitoring. (Lambert 2003) This project was examining physical health for people in the community with a mental illness. We changed our approach to shared care “everyone’s business initiative “. (Queensland Health 2015)This brought extensive buy in from General Practitioners (GPs) who we developed a handbook for, guidelines and training for the GPs.
It changed the approach to how community nurses conducted their day to day business by including a wider scope of physical health assessments. It monitored the patients physical needs, provided referrals for any adverse results obtained from investigations. It also provided a pathway to discharge from the service or facilitate “shared care”. It opened lines of communication to provide more comprehensive care for patients. Community nurses educated and mentored the patients that had complex needs. The monitoring documentation was developed to observe the base line and start of open episode to the community (CIMHA 2008). This recorded the first weight in kg, BMI, waist (girth), blood pressure, lipids, liver function, prolactin levels, Lithium, Valporate, Clozapine, and Carbamazepine. (Queensland Health 2007)
This monitoring is captured on an electronic record. It really set a standard for looking at health and well-being and developed the pathway for signs of chronic disease. (American Diabetes Association 2004) We changed our approach to look at green(healthy), amber (monitor closer) and red (dangerous levels) and addressed high risk or signs of chronic disease. We have now added this monitoring to the community case reviews every 3 months
